Senior Practitioner



An excellent opportunity has arisen for an experienced Senior Practitioner to oversee Specialist Social Care interventions to Stroke patients and their carers at all stages within the Stroke Care Pathway.



The purpose of the role will be to:

-Provide advanced social work support to complex stroke cases, including Adult Protection situations when required;

-Lead on a social care focus on delivering a patient centred approach to rehabilitation of stroke patients;

-Assist in the development of interdisciplinary working in provision of stroke care, working towards integration with health services;

-Identify needs of people who have suffered a stroke and work with them to formulate care plans, identify suitable resources and negotiate with providers;

-Assess and review care packages;

-Supervise Qualified and Unqualified Social Workers



The ideal candidate will have:

-A thorough understanding of the needs of adults who have suffered a stroke;

-Substantial experience of assessment and care management;

-Advanced social work and care management skills, including multi-agency working, especially with health and therapy colleagues;

-Detailed knowledge of relevant legislation;

-Excellent written and verbal communication skills;

-Ability to use I.T. within the context of this role;

-Ability to supervise, motivate and support others;

-Full satellite file available on request, including a copy of the candidate`s CRB
